The Community & Provider Relations Coordinator serves as a visible and trusted representative of Jenkins County Medical Center, strengthening relationships with provider practices and community partners to support referral growth, outreach efforts, and hospital initiatives. This role requires strong written and verbal communication skills, an engaging and professional presence, and the ability to learn hospital services quickly in order to represent them accurately and consistently.

Key Responsibilities
Community Outreach & Georgia HEART
- Represent Jenkins County Medical Center at community events, health fairs, chamber activities, and civic meetings.
- Develop and maintain relationships with local businesses, schools, churches, and community organizations.
- Promote awareness of the Georgia HEART Hospital Program through community outreach, presentations, and conversations, in coordination with the Marketing Director.
- Assist with outreach efforts that encourage community members and businesses to support the hospital through Georgia HEART participation.
- Help coordinate hospital-hosted events, open houses, and educational programs that strengthen community trust and engagement.
- Oversee community health education aligned with CHNA strategic initiatives and facilitate the Community Health Needs Assessment (CHNA) every three years to ensure compliance with IRS 501(c)(3) / 501(r) requirements.
Provider Outreach & Referral Relations
- Build and maintain positive working relationships with physicians, advanced practice providers, clinic managers, and referral partners.
- Conduct regular outreach to provider practices to share information about hospital services, programs, and referral pathways.
- Serve as a liaison between provider practices and hospital departments to support clear communication related to referrals and services.
- Assist with identifying referral opportunities by understanding provider needs and hospital capabilities.
- Collaborate with the Utilization Review Manager to identify provider needs, referral trends, and service gaps, supporting appropriate utilization and improved care coordination.
- Support provider appreciation and recognition initiatives.
Marketing & Communication Support
- Write clear, accurate, and engaging content for outreach materials, emails, letters, presentations, and other communications.
- Work closely with the Marketing Director to learn hospital services, initiatives, and messaging and communicate them consistently.
- Assist with social media content and engagement as directed.
- Provide support for employee events and internal activities as needed.
- Support the hospital foundation through assistance with events, outreach, and donor relations as needed.
Qualifications
Education and Experience
- Associate or bachelor’s degree in a related field preferred, or equivalent experience in communications, outreach, or provider relations.
- Experience in healthcare outreach, communications, provider relations, or community-facing roles is preferred.
Skills and Abilities
- Excellent written and verbal communication skills.
- Engaging, personable, and comfortable building relationships with providers and community members.
- Quick learner with the ability to understand and communicate new information accurately.
- Organized, dependable, and attentive to detail.
- Proficient in standard computer applications, including Microsoft Word, Excel, and Outlook; experience using graphic design tools is a plus.
- Valid driver’s license and ability to travel locally.
Work Schedule
- Primarily weekday hours with occasional evenings or weekends for community events.
